Training Request Form

Hi Everyone!You have all been so very helpful in the past, I thought I'd see what kind of feedback you have for me this time.I wonder if any of you have created training request forms. I need two forms (or one that can be used for both - however I think two forms would be better) 1. to request development of an eLearning course2. to request to take training either in person or online within our organization. (this one is pretty easy I've got it almost done)I'm looking for bits and pieces that should be included or maybe things that you wish your forms had so that I can make my form something that doesn't have to be edited for a very long time.Any words of wisdom would be greatly appreciated.
